Ishar 2: Messengers of Doom continues the series with a narrative centered on a new threat to the region. After the original fortress became a hub of civilization, a monk named Shandar exploited trade in mind-altering substances to gain control of the developing city of Zach's Island. His rise to power, which includes constructing a rival fortress, prompts the warrior Zubaran to embark on a quest to stop him.

The game expands upon its predecessor's framework, offering a larger world to explore that features 3D environments such as dungeons, settlements built among trees, and mountainous terrain. A day and night cycle influences activity, with certain establishments like shops and nightclubs operating on specific schedules. Players manage a party, engaging in real-time combat and exploration, with the option to import characters from the first game or recruit new ones.

Additional gameplay elements introduced in this sequel include the utilization of domesticated animals to assist in fights and the crafting of magical potions. The core systems of party management and real-time combat remain consistent, set within a fantasy landscape that has grown in scale and interactive detail.
