Duke Nukem: Episode 1 Shrapnel City is a 2D scrolling action game where the player guides Duke Nukem through levels. The core task involves navigating to a level's exit, a goal that requires overcoming enemies and interacting with the environment. Various on-screen objects, from blocks to obstacles, can be destroyed by shooting them.

Progression is supported by collecting items scattered throughout the stages. These include points, health restoratives, and weapon enhancements. Certain special inventory items provide unique abilities. The structure of each episode culminates in a final level that diverges from the standard format, replacing the exit with a boss encounter against Dr. Proton that must be won to complete the episode.

A scoring system offers additional challenges beyond simple completion. After finishing most levels, players can earn substantial point bonuses for accomplishing specific in-level achievements, such as destroying every surveillance camera. This bonus mechanic was also utilized in later Apogee titles, including the direct sequel Duke Nukem II. The series eventually evolved from this 2D foundation into a 3D format with a subsequent release.
