Duke Nukem 3D on the Game.Com adapts the first-person shooter into a grid-based dungeon crawler due to the platform's constraints. Movement is restricted to stepping forward, backward, or strafing one screen at a time, with separate buttons used to fire left or right without changing Duke's facing direction.

The content is organized into three episodes, containing twelve missions that are simplified versions of the original levels. Progression is managed through passwords issued upon the player's death. Available equipment includes four weapon types and collectible items such as armor, key cards, health, and ammo.

Opposition comes from four distinct enemy types. The experience includes digitized voice lines from the protagonist and offers four selectable difficulty settings, making the challenge adjustable.
