Visual and animation updates distinguish this version, with examples being an animated coin toss and new player movements. On the field, strategic options are expanded with additions like a no huddle offense and the ability to stop the game clock during play.

Beyond the contemporary teams, the roster is supplemented with two All Madden teams and the eight Greatest Ever teams. The Sega Genesis iteration of the game incorporates digitized speech from John Madden and uses battery backed memory to save playoff outcomes and player statistics.
