Dragon Quest Monsters: Caravan Heart is a Japan exclusive Game Boy Advance title that continues the series' focus on monster collection and training. The player controls Kiefer, a prince from Dragon Quest VII, who is transported to the land of Alefgard and must lead a traveling caravan to find a cure for a local boy's parents.

Gameplay revolves around managing this caravan, which serves as a mobile headquarters. Travel consumes food rations, adding a resource management layer to exploration. The party can include both monsters and human characters, with the latter able to assume different jobs. A key system involves merging monsters using a resource called monster hearts to create more powerful allies, replacing the breeding mechanics of earlier series entries.

The narrative is a prequel to the events of Dragon Quest VII. Kiefer's journey, initiated by the goddess Rubiss, requires him to locate the Ball of Roto artifact, which is needed both to heal the sick parents and to secure his own return home.
