Ultimate Mortal Kombat 3 is a 2D fighting game supporting both single-player and multiplayer competition. Its narrative centers on the conflict between Earthrealm's defenders and the Outworld forces led by Emperor Shao Kahn, who has merged the two realms.

The combat system is built around a five-button configuration for different punches, kicks, and blocking. A dedicated run button facilitates aggressive play, and preset chain combos are a key mechanic. Interactive stages feature elements like floor transitions and lethal environmental hazards known as Death Traps.

Players choose from an initial selection of 23 combatants, each possessing unique special and finishing moves. The roster includes series regulars like Scorpion and Sub-Zero, with additional hidden characters such as Mileena and Ermac accessible through specific codes or gameplay achievements. The variety of match-ending sequences, including Fatalities and Babalities, remains a signature feature.

Single-player progression involves a challenging tournament ladder where opponents increase in difficulty, culminating in battles against bosses Shang Tsung, Motaro, and Shao Kahn. Multiplayer options extend to direct versus matches, two-on-two team battles, and an 8-Fighter Tournament mode. Various difficulty settings and match rules can be adjusted.

Later adaptations for portable systems like the Game Boy Advance and Nintendo DS offered altered versions of the experience, with the DS release incorporating wireless multiplayer and the Puzzle Kombat mini-game.
