Splatterhouse 2 is a side-scrolling action game structured across eight distinct stages. The player controls Rick, who is driven by a recurring nightmare to return to the horrific setting in an attempt to rescue Jennifer.

Combat involves a foundational set of unarmed attacks, which can be supplemented by various found weapons. These armaments range from blunt objects to more destructive tools like a shotgun or chainsaw. Each stage concludes with a confrontation against a unique boss, where success typically requires learning and executing a challenging specific pattern of moves.

The narrative premise explains that Rick's return is facilitated by the same terror mask from the first game, which grants him enhanced, rage-fueled power at a moral cost. This concept, along with the game's visceral tone, shows a clear influence from horror cinema. Enemies encountered throughout the journey include a variety of grotesque creatures, from animated skulls to large demons.
