Back to the Future Part III for the Sega Genesis adapts the film's 1885 setting into a series of four distinct arcade challenges. The narrative involves Marty McFly and Doc Brown attempting to escape the era while contending with the outlaw Mad Dog Tannen and his gang.

Gameplay shifts between different perspectives and mechanics across its segments. One sequence is a side-scrolling and top-down chase on horseback, requiring evasion and shooting. Another presents a straightforward target shooting gallery with hidden elements.

Confrontations with the gang take an isometric form where players throw pies at armed opponents who return fire.
