WWF Betrayal is a side scrolling brawler for the Game Boy Color that features licensed World Wrestling Federation personalities. The core gameplay involves navigating through six distinct levels, where the player character combats a variety of foes including generic thugs and fellow wrestlers. Attacks consist of basic strikes and the use of environmental weapons like pipes and wooden planks.

Players select one of four starting characters: The Rock, Triple H, The Undertaker, or Steve Austin. The narrative premise involves a backstage attack that costs the wrestler a championship match, followed by a mission to rescue Stephanie McMahon at the behest of Vince McMahon. This rescue operation forms the objective across the game's stages.

Level traversal presents environmental hazards such as fire and floor traps alongside enemy encounters. Defeated opponents may drop power ups that restore health or a special power meter. When this meter is full, the wrestler can execute a signature finishing move, such as the Pedigree or Stone Cold Stunner, on a targeted enemy.

A second player can participate in the campaign through the use of a link cable, enabling cooperative play. The game's structure requires managing both combat and environmental navigation, which can be challenging.
