E.T. the Extra-Terrestrial and the Cosmic Garden for Game Boy Color centers on cultivating alien flora. The player assists E.T. in a botanical project aboard his ship, aiming to finish a garden to facilitate his return home.

Gameplay involves managing resources that deplete and cross-pollinating seedlings to generate new plant types. Over sixty varieties can be created, some exhibiting unusual behaviors like movement or emitting energy. The player is aided by two companions, a Space Bee and a Space Slug.

The task is presented as challenging, with adverse conditions and unexpected situations across twelve levels set on six planets within seven environments. Successfully producing a required number of plants unlocks a final, perpetual Prize Garden.
