MLB SlugFest 20-04 brings Midway's arcade-style baseball to the Game Boy Advance, marking the series' sole appearance on the platform. It utilizes the official teams and players from the 2003 MLB season, with six different stadiums represented.

The presentation employs two-dimensional graphics, and the perspective shifts depending on the action. While pitching and batting are handled from a view behind the batter, fielding is viewed from an isometric angle. The game incorporates signature Midway elements, such as a turbo meter for enhanced player actions and an on-fire state that boosts abilities after a string of successful plays.

Available modes are more limited than in console versions. Players can engage in a single Quick Play exhibition, a Challenge mode that functions as a condensed season against every team, or a best-of-seven Series mode. Progress in the latter two is recorded using a password save system.
