Elevator Action: Old & New for the Game Boy Advance packages two distinct versions of the same core game. The player controls a spy tasked with infiltrating a multi-story building to retrieve documents and escape, navigating between floors using elevators.

Gameplay involves moving through a side-view, non-scrolling environment while evading or confronting guards. Tactical options include shooting enemies and shooting out lights to temporarily hinder opponents in darkened areas.

The "old" portion is a direct re-release of the original title. The "new" arranged version retains the fundamental mechanics but presents them with altered, more cartoonish visuals and expanded level designs. This version introduces additional challenges, new enemy types like robots and zombies, a health bar system, and multiple playable characters with unique attributes.
