The Legacy: Realm of Terror is a first-person game set within a mansion besieged by supernatural forces. The player's objective is to investigate the dark history of the Winthrop family and survive the horrors now inhabiting the estate.

Gameplay blends role-playing and adventure mechanics. Exploration of the 3D environments is necessary to find items and solve puzzles. Character progression is a core element, with a choice of eight protagonists whose abilities fall into physical, supportive, and combat skill categories.

Combat occurs in real time within the exploration view, requiring quick selection of weapon or magic commands. Spells, which are found as pages and can be upgraded, provide offensive, supportive, and restorative options. Managing the character's condition also involves activities like resting and consuming food.
