R-Type III: The Third Lightning is a side-scrolling shooter where the player controls an R-90 spacecraft against the returning Bydo Empire. The objective involves progressing through six stages to confront a final mothership target.

Its systems build upon the series' established mechanics, featuring large levels and boss encounters. A central element is a detachable laser unit that can be positioned to fire forward or backward. A new addition allows for the selection of one of three distinct unit types at the game's start, each of which determines the specific laser behaviors unlocked through collected power-ups.
