Battletoads in Battlemaniacs is a sequel that continues the series' hybrid approach to gameplay. It incorporates combat sequences reminiscent of beat 'em up titles alongside other distinct stage types, such as racing segments and levels involving mounted creatures.

The narrative setup involves the three protagonists, Zitz, Rash, and Pimple, attending a demonstration in Tibet for a virtual reality device called the Psicone Gamescape. When a creature emerges from the simulation to abduct both Zitz and the CEO's daughter, the remaining two Battletoads must venture into the virtual world to stage a rescue.

Players have the option to experience the game cooperatively, with two distinct modes governing whether friendly fire between characters is permitted. The progression includes occasional bonus stages that provide opportunities to earn additional lives, which suggests a challenging overall pace.
