The Revenge of Shinobi is a sequel where the player character, Joe Musashi, seeks vengeance against the reformed Neo Zeed organization. This group has targeted his clan, resulting in the death of his master and the capture of his bride.

Gameplay involves navigating through distinct environmental zones, each containing three levels and concluding with a boss encounter. The journey progresses from rural and forest settings to urban, industrial, and transportation hubs, culminating on an island. Enemy types are often tied to these locations, ranging from soldiers with period weaponry to various kinds of ninjas.

Players primarily attack using throwing knives, which can be upgraded to fire projectiles and grant a sword by collecting power packs found in destructible boxes. These containers can also yield extra weapons or health, though some are rigged with explosives. The core objective in each stage is to defeat adversaries and locate the exit.
