Roger Clemens' MVP Baseball is a baseball simulation for the Sega Genesis featuring fictional teams and players, with the notable exception of Roger Clemens himself, who appears on the Boston Hornets roster. The game offers two primary modes: a single exhibition match and a full Regular Season mode that includes playoffs and tracks progress via an in-game calendar.

Gameplay involves direct control over pitching, batting, and fielding actions. Pitching allows for manipulation of speed, spin, and placement, while batting requires precise timing to make contact. Defensive play shifts to a lower camera angle for fielding, with the player controlling the nearest fielder to retrieve the ball and make a throw. The camera system dynamically selects perspectives based on the ball's location, panning to follow the action from various infield and outfield positions.

Teams are organized into four fictional divisions, such as the National Division West with the San Francisco Quakes or the American Division East with the Detroit Wheels. Each team comprises players with individual skill ratings. The game supports two-player exhibition matches and uses a password system to save progress in the season mode.
