Muhammad Ali Heavyweight Boxing is a sports simulation for the Sega Genesis that centers on the sport of boxing. The presentation combines two-dimensional character sprites with a three-dimensional ring environment, allowing fighters to move freely in any direction. Players can select from a roster that includes the titular champion and nine other fictional boxers.

Match parameters are highly customizable, with options for round count, duration, and knockout conditions. Two primary modes are available: a standard exhibition match for solo or versus play, and a tournament for single players. The control scheme offers a choice between a simplified arcade style and a more detailed simulation. The simulation mode provides direct control over specific punches and defensive maneuvers assigned to separate buttons and directional inputs.

Fighter performance is governed by dynamic power and speed bars that deplete during exertion or when taking damage, affecting punch strength and velocity. These attributes can recover through defensive play. A punch counter for each fighter is displayed, and it serves as the sole criterion for judges' decisions in the event of a match going the distance. The game includes digitized audio, featuring samples of Muhammad Ali's voice.
