The Flintstones: King Rock Treasure Island is a side-scrolling platform game where Fred Flintstone seeks treasure after discovering a map. His journey consists of seven stages, some concluding with a boss encounter.

Gameplay involves navigating platforms and avoiding hazards. Fred uses thrown axes as his primary weapon, which stun enemies rather than eliminate them immediately. A stunned foe can be used as a temporary platform, and defeating it permanently requires a second hit while it is dazed. However, enemies reappear if Fred retreats to their initial position or near specific skull markers, which often facilitate progression by creating makeshift staircases.

Collectible items are present, with most serving to replenish health or add to the score. One notable power-up summons Fred's dinosaur, Dino, as a mount. Riding Dino alters mechanics, enabling straight-axis throws and enhanced jumping ability, but the mount is lost after a single hit.
