Bishoujo Senshi Sailor Moon R for the Game Boy adapts the Sailor Moon R storyline, featuring confrontations with the Black Moon Clan. The central characters include Usagi Tsukino, her future daughter Chibiusa, and the other four Sailor Scouts.

Gameplay is divided into two distinct modes. The primary Story Mode involves guiding Usagi and her companion Luna through Tokyo, where progression requires interacting with characters, visiting different areas, and completing various mini-games. This mode assigns control of a specific Sailor Scout or Chibiusa for each chapter.

Both modes incorporate action-oriented platforming stages populated by enemies and concluding with a boss encounter. An alternative Arcade Mode allows immediate access to these combat levels with any of the five main Scouts, bypassing the narrative and exploration segments entirely.
