Test Drive: Off-Road 3 on Game Boy Color is a top-down racing game featuring licensed off-road trucks from several major manufacturers. Its primary structure involves progressing through a series of cups in Tournament Mode, which sequentially unlocks new tracks and provides currency for vehicle purchases.

Vehicle performance is customizable using points earned after races, allowing upgrades to acceleration, top speed, traction, and gearing. Additional equipment includes a GPS for track navigation and special items like speed bursts and recovery winches for overcoming obstacles. Beyond the main tournament, players can engage in single races or a unique mode where police vehicle variants pursue and ticket other racers.

The game includes a selection of international environments, each with multiple tracks, and offers hidden content such as bonus cars and courses. A rumble feature is present in the US cartridge version, providing physical feedback from the terrain.
