The Grim Adventures of Billy & Mandy adapts its cartoon source into a side-scrolling action game. The core objective involves retrieving scattered Mojo Balls, which cause supernatural rage, to prevent universal chaos.

Gameplay combines platform navigation with combat sequences. Players utilize basic attacks, a special move powered by a meter filled from defeated enemies, and mobility options like a double jump. Progress in the main Story mode requires clearing areas of all foes before advancing to a boss, whose defeat yields a Mojo Ball.

Character progression is tied to these collectibles. Initially only Billy is playable; completing his story and gathering Mojo Balls unlocks Mandy, and subsequently Grim. Many Mojo Balls are earned in a separate Mission mode, featuring short, often time-limited stages that function like mini-games.
