Monsters, Inc. for the Game Boy Color is a side scrolling platform game adapting the film's narrative. Players navigate through twelve stages, beginning with a tutorial and moving through settings such as the scream factory, various bedrooms, city streets, and a Himalayan area.

Control alternates between the two main characters, Sulley and Mike, who possess distinct skills. Sulley can move objects to create platforms, break certain walls with a dash, and cling to edges. Mike is capable of a double jump and fitting into small openings. Their offensive options differ as well, with Mike throwing cola cans and Sulley hurling toys.

Level objectives vary, requiring either a simple exit, the collection of specific items, or the completion of tasks like frightening a set number of children. Hazards include contact with toys or, in later stages, other monsters. The pursuit by Child Detection Agency agents introduces a challenging threat, as their touch results in an immediate life loss.

A unified health and ammunition system exists where collecting Blort cola cans serves both purposes. Additional collectibles include Chinese takeout boxes, where gathering ten grants an extra life, and teddy bears that provide an instant life bonus.
