The story begins when a strange event on Hachi Isle causes Hachiemon's friends to lose their lips, making basic functions difficult. His quest is to recover them. This premise is tied to the character's origin as a mascot for the TV station KTV, also known as Kantele, which is referenced in the game through voice clips.

Gameplay revolves around using the lips as a multi-purpose tool. They can be thrown as a projectile, used to grab onto surfaces for traversal, or even to stun certain enemies with a brief kiss. Additional actions include a rolling attack that can cause dizziness if overused. The journey involves navigating past various enemies, obstacles, and boss encounters.

The character is voiced, delivering comically deep and expressionless comments for actions like pausing, attacking, or taking damage. These audio cues, along with exclamations of "Kantele," contribute to the game's presentation.
