Earthworm Jim presents a platforming experience that builds upon the established mechanics of its SNES predecessors. The gameplay involves navigating through a series of levels set within an unconventional domestic environment.

Players control the protagonist through twelve stages located in his own bedroom, confronting a variety of antagonistic creatures. While the threat level remains significant, this scenario differs from his prior conflicts with foes like Psy-Crow. The arsenal includes many familiar, unconventional attacks supplemented with new additions, and a recurring character, Professor Monkey-for-a-Head, makes an appearance.
