Altered Beast: Guardian of the Realms is a side scrolling action title where the player character, resurrected by Zeus, fights through numerous monsters across fifteen distinct realms. The objective in each area is to reach and overcome a final boss.

Combat involves standard punches and kicks, with defensive options like jumping and ducking. Throughout the levels, players can collect various attack items that grant temporary advantages such as healing, protective shields, or a increased speed.

A multiplayer feature allows for linked battles using creatures gathered during the single player story. The core narrative conflict involves stopping a monster named Arcanon.

At the beginning of each level you are a little weakling ;)
Fight some zombies.
Fight some skeleton axe battlers.
Fight some minotaurs.
The purple wizards protect the red wizard.
If you beat red wizard, he will drop an orb.
After you take the orb you will become stronger.
The next orb is right above our hero.
After taking the second orb, we get even stronger.
With the 3rd orb we transform into a monster, here the wolf.
The wolf's rush attack is very powerful.
