Commander Keen 6: Aliens Ate my Baby Sitter centers on the rescue mission of eight-year-old genius Billy Blaze, whose babysitter Molly has been taken to the planet Fribbulus Xax by the Bloogs. The player navigates this world through a top-down map that serves as a hub for selecting individual stages, though accessing certain areas requires locating specific tools first.

Gameplay involves guiding Billy through side-scrolling levels filled with environmental dangers and hostile creatures. These include Blooguards, Flects that can deflect his attacks, and charging Nospikes. Contact with these foes or environmental hazards results in the loss of a life, making the overall experience challenging.

Billy is equipped with a raygun to neutralize most threats and a pogo stick for enhanced mobility. Exploration yields various collectibles, such as ammunition, gems to unlock doors, and score-boosting items. Finding bird-like Vivas can grant extra lives, with a Queen Viva providing one immediately.
